2022 Vaccine Clinic: COVID, Flu, Pneumonia, Shingles, Tetanus, and Hepatitis-B. Co-sponsored event with Chevy Chase Village. Pre-registration and completion of a consent form are required prior to vaccination. To complete a consent form online, visit www.chevychasevillagemd.gov and select from the homepage "Register for the October 19 Vaccine Clinic". Hard copies of the consent form will also be available at the Village Hall. Most vaccines will be covered by insurance. Participants are responsible for eligible co-pay per their medical insurance coverage. Participants must present their medical insurance/Medicare card and ID at the time of vaccination for medical insurance billing.
